How to update your child's Medical Information form?
Have you been asked to update your child's Medical Information form?
Even if there are no changes, we must request regular audits of the data being held, and by carrying out the process below, it will ensure our data remains current. We thank you advance for your assistance.
To access your son's medical information, please follow the instructions below:
Open https://home.scotch.wa.edu.au, search for Medical using the search bar and click on the tile.
Sign in with your Parent ID (refer to the image below).
Note your Parent ID follows the format as shown in the example below.
ParentID@parent.scotch.wa.edu.au (this change came into affect July 2023)
Here is an example: s123456@parent.scotch.wa.edu.au

Note - you do not use your personal email address for accessing Scotch online resources.
If you encounter the message 'your username is valid but does not match our records', you must clear your browsing data or open an incognito/private browsing window.
How do I do this?
Chrome - File > Open Incognito) or in Safari, File > Open Private Window.
Select Medical Information Check or Contact Information Check, depending on the information you wish to update.
IMPORTANT: Please review the information on the Medical Information Form. You cannot edit the form; you must add changes/additions in the Text Box at the bottom.
Select YES and SEND to Health Centre if the medical information is correct.
Otherwise, any corrections or instructions should be typed in the Text Box.
Once you have completed your changes, click 'SEND to Health Centre'. An email will be received from medicalOnline@scotch.wa.edu.au confirming receipt of your changes. Receipt of that email ensures that you have completed the process. The Health Centre staff will update the database according to your instructions.
The Tech Centre can assist you with login issues but cannot complete the medical form on your behalf.
